General anesthesia is the culprit when determining how long your downtime might be. When compared to the advanced local anesthesia techniques we have developed and have used exclusively over the last decade, downtime has been reduced to weeks rather than a month, and costs have been drastically reduced while delivering comparable results as when performed under general anesthesia.
Dr. Malcolm Lesavoy’s facial rejuvenation procedures are done in the privacy of his office under oral sedation using his advanced local anesthetic techniques that offer surprising comfort and provide a solid skin numbing anesthesia for hours. Most patients comfortably snooze through the procedure as their rhytidectomy is meticulously performed to tighten the SMAS and skin.
A general road map to Dr. Lesavoy’s Luxury Lift includes the judicious use of epinephrine in the different local anesthetics that keep bleeding to a minimum. The incisions are kept as short as possible reminiscent of what are called mini-facelift incisions. The incisions are carefully hidden in and around the ear with special attention given to the tragus (the cartilage on the front part of the ear) and the earlobe. Depending on the patient’s earlobe condition pre-operatively, the earlobe may require reshaping and re-piercing after reconstruction to restore a more youthful and natural shape.
The skin is undermined approximately 3-4 cm to gain access to the SMAS which is the basic structure that is used to apply tension (tightening) for the cheek, jawline, and neck. The direction of pull on the SMAS, or vector, is different for each part of the face. When the SMAS is plicated, it makes the undermined area of skin much smaller (1-2cm). The skin is then undermined again, and a second layer of permanent plication sutures are placed. The undermined area of skin, called dead space, is reduced just to the point where the extra skin is excised leaving only 2 cm of dead space where most facial bruising emanates. Most typical general anesthesia facelifts leave at least 4 cm of dead space and generally have more bruising, swelling, and run a higher risk for hematomas.

Local Anesthesia Face Lift and Eyelid Surgery
On some rhytidectomy patients, even if they have had a previous lower blepharoplasty, Dr. Lesavoy may recommend lower blepharoplasty which concentrates of redistribution of the fat and blending the lower lid skin with the cheek skin which already has been elevated and contoured with fat. This lower eyelid-cheek blending is ideal for resolving dark circles at the lower eyelid-cheek junction.
Dr. Lesavoy’s face surgery patients are treated with individual care as their facial rejuvenation is performed in a matter of a few hours and can go home following surgery. Sutures are usually removed approximately 10-14 days following the face procedure, but patients should still expect to take care of the surgical site.

Types of Rhytidectomy performed with Local Anesthesia Lesavoy Luxury Lift
- Mini Face Lift – this face surgery is typically a shorter version of a regular rhytidectomy that includes shorter incisions. This procedure is usually for the patient that needs facial rejuvenation of the face that has not approached a dramatic degree of aging. The mini lift includes smoothing of the skin and tightening of the SMAS layer of muscle.
- Facelift – The most typical face lift is the generalized rhytidectomy that includes tightening of the skin and SMAS layer of muscle. The general rhytidectomy addresses sagging skin of the face including the jowls, cheeks, and nasolabial folds. This procedure addresses facial rejuvenation of the mid and lower face areas.
- Deep Plane Facelift – deep plane lift is a regular rhytidectomy but with a different approach than the traditional facelift. Like a regular lift, the skin and SMAS are tightened but the approach is below the SMAS system of the mid-face. Despite many trending claims, this procedure has been around since 1990 and is not new to real board-certified plastic surgeons like Dr. Lesavoy. This type of rhytidectomy addresses the mid and lower face areas to achieve facial rejuvenation.
- Mid Face Lift – the mid lift is a rhytidectomy that addresses facial rejuvenation of the cheek area of the face. This procedure is for the facial rejuvenation patient that presents with concerns for the cheeks and nasolabial fold areas of the face. This procedure involves elevating the malar pads (cheeks) to rejuvenate the signs of facial aging and involves tightening of the SMAS and skin.
The final result must look natural, and not look fake or “done.” After the healing process, friends and family should ask “is your hair different, have you changed your diet, did you just come back from vacation, are you working out more?” and so on.

When can I return to work after surgery?
Most of Dr. Lesavoy’s rhytidectomy patients return to work between one and two weeks following their procedure. Your face will continue with the healing process for approximately 6 months as the incisions continue to mature and fade.
When can I wear earrings again after face lift surgery?
Most patients can resume wearing earrings about 2 weeks after their procedure. However, please keep in mind that heavy earrings are not allowed in the beginning.
What other procedures can be performed at the same time as a face lift?
Since no face lift is one-size-fits-all and not all facial rejuvenation patients need all facial procedures to address their cosmetic goals, Dr. Lesavoy addresses each patient’s concerns and creates an individualized facial rejuvenation plan. The most common ancillary cosmetic procedures performed with a rhytidectomy are eyelid lift, brow lift, and neck lift. During your consultation with Dr. Lesavoy, you will be able to discuss your facial rejuvenation concerns and he will develop a facial rejuvenation plan to help you meet your cosmetic needs.
What can I expect the day of my facelift procedure?
Patients are asked not to drive themselves to the facility on the day of their surgery. Dr. Lesavoy requests you shower the day before or the morning of surgery prior to arriving at the facility.
How do I prepare for face surgery?
Preparing for your cosmetic face surgery is something for you to begin a few weeks prior to your scheduled surgery date. Dr. Lesavoy will review your medical history prior to surgery to give you specifics accordingly, but here are some universal pre-operative surgery instructions:
- Hair care – since you will not be able to wash your hair for a few days following your procedure, Dr. Lesavoy requests you wash your hair the morning of the day before your procedure. Additionally, patients should also take note you will not be allowed to get your hair colored or permed for approximately six weeks following your procedure during your recovery.
- Cease Nicotine & Alcohol Use – Beginning one month prior to your scheduled surgery, you should cease use of any nicotine products including cigarettes, nicotine patches, and vaping. Nicotine is a medical known problem when it comes to healing as it decreases circulation and slows healing time. Likewise, alcohol use also slows healing time, increases bleeding during surgery and interferes with medications.
- Cease NSAIDS – Patients should discontinue use of all NSAIDS two weeks prior to their scheduled surgery date. NSAIDS include medications such as aspirin, BC Powder, Aleve, Advil, ibuprofen, Motrin, naproxen, and many decongestant medications. NSAIDS are also blood thinners and cause surgical patients to bruise and swell more. Since the goal here is to reduce your recovery time, cessation of NSAIDs is vital to reducing bleeding during the procedure and post-operative bruising.

What can I expect with local anesthesia facelift recovery?
Dr. Lesavoy requests his patients to have someone care for them for the first 24 to 72 hours after surgery. Having a positive and supportive friend or family member to assist you in your cosmetic surgery journey is a key component to your recovery.
Most patients report less than expected pain, bruising, and swelling with the Lesavoy Luxury when they follow Dr. Lesavoy’s recovery instructions:
- Sleep elevated the first few nights. Sleeping with plenty of pillows and even using a travel neck pillow is highly recommended.
- Plan to eat soft food for the first few days. Some great post operative favorites are yogurt, fresh fruit, and low sodium soups.
- Prepare some cold packs to help reduce swelling. “Frozen peas” has been a patient favorite for many years. You may use the cold packs 20 minutes on and 20 minutes off while awake for the first 48 hours.
- Do not plan to shower for about 4-5 days after surgery. Dr. Lesavoy will let you know when you will be allowed to shower. When you do, please be very careful washing your hair, paying extra careful attention behind and in front of your ear along the incisions. Baby shampoo is highly recommended instead of your usual shampoo to avoid irritation and burning. Steam baths and saunas are not permitted for a few months following your surgery.
- Use of lubricant eye drops is highly recommended especially if you also had blepharoplasty with your facelift. If your eyes are watering, that means they are dry and need hydration.
- Keep sutures clean, but do not scrub them. The sutures will be removed between one to two weeks following your procedure. Please do not use makeup on your sutures until Dr. Lesavoy gives you the okay to do so. Use of makeup on your sutures can cause an infection.
- Patients should use sunglasses and other sun protection following their procedure. Most patients shade their faces and incisions with the use of scarves and hats. Use of sunscreen is mandatory for up to six months following your procedure. Dr. Lesavoy will instruct you as to when you will be allowed to use a SPF of 30 or greater directly on your incisions.

Very few real Board Certified Plastic Surgeons do the face and neck and eyelid surgery under local anesthesia. Dr. Lesavoy has been doing this for approximately 15 years and found that patients recover much faster than with general anesthesia. However, if a patient insists on having their neck and face and eyelid surgery under general anesthesia, Dr. Lesavoy has no problem with it. General anesthesia is very safe; however, you will never have an untoward result from general anesthesia if it is not used.
